CASE MANAGEMENT SERVICES
WHAT: Case management services are designed to facilitate access to services
and coordinate care among child serving systems. This promotes smooth transition
between levels of care within CGRC and within the continuum of children's mental
health services, county wide.
Case management is provided for children from 0 to 21 years of age, Monday through
Friday, 9:00am to 5:00pm. |
